Now Pay $0 (zero dollars) for your prescription eyeglasses and prescription sunglasses. We can bill your entire purchase amount to your Insurance company directly.
Upon payment from your insurance company, we will process and ship you your order.
Here are the steps you need to follow for Insurance Submissions:
1. Select the Frame and Lens package you like to order. Use coupon code "Insurance" during checkout to pay $0 out of pocket.
2. Attach a picture of your most recent Prescription, A valid Piece of ID (ie. drivers license), and Your Insurance membership Card.
3. In the notes, please write to us your Full name, Address, Date of Birth, Insurance company name, your policy number, and your member ID. If you are not the primary insurance plan holder, then we also need the primary holder's full name, date of birth, and relationship to you.
Once we have received all of the above information, we will then submit your claim to your insurance company. Please make sure you are signed up for online billing with your insurance company. If not, please call your insurance company and request that you are permitted for online billing.
Once your insurance submission has been completed, we will delete your personal and insurance information from our system.
If you decide to claim your purchase directly from your insurance company yourself, then simply go through the checkout process, pay for your purchase, and print off the official PDF copy of your receipt, generated automatically by our system, to submit to your insurance company. We are fully authorized by all insurance companies to fulfill your prescription glasses, and can confirm your purchase if contacted by your insurance company.
Please note that only prescription glasses and prescription sunglasses are eligible for insurance submissions. We cannot bill your insurance company if your order is for non-prescription glasses or sunglasses.
